Useful Notes

The data for this study have been converted from Binary format to other data 
formats.  Variable and some value labels have been added to aid use.

A look-up table has been added to the PDF file, listing the original 
questionnaire page number, variable number and card.  It also lists the new 
variable number in the data file.

Please see the documentation for the coding structure of any variables without 
value labels.

Please note that columns 15-20 on card 8 variable DOB (Date of birth) runs ddmmyy for 
the majority of data but has been entered in reverse for some cases, i.e. yymmdd.
